Registering with the Practice
Please bring your medical card if possible when registering, as it contains your NHS number. If you do not have your card, simply fill out a Patient Registration Form and Health Questionnaire, putting as many details on as possible.

You will also be required to bring two forms of identification with you:
- a passport or Full driving Licence (both parts)
- a utility bill with your name and address on it
An appointment for a health check with a Nurse needs to be made for patients over 6 years. A Doctor may need to see them before they can be registered. A urine bottle will be given to you to bring a sample when attending the health check.
Allocated Named GP
Please note that all patients now have an Allocated Named GP. This does not affect who you wish to see when making an appointment. You can request the name of your Allocated GP at our Reception.
A practice booklet will also be available when you register.
We are always happy to see any relatives or friends staying with you if they need a Doctor or Nurse. They will need to fill out a short form for the information to be returned to their own Doctor. Please arrive 10 minutes before your appointment to allow time for completion of the necessary forms.
